Choosing a Facility

Basic Information:

**Items we suggest you look for when choosing a Skilled Nursing Facility (SNF) for your loved one:

The SNF has beds accessible and gives you the highest quality care that each resident deserves.

The SNF offers special services for each individual in a separate unit including a ventilator or rehabilitation with a bed available.

The SNF is located in an area that is close to family and friends.

Resident Appearance:

Each resident is groomed well, clean and dressed appropriately for the season.

Living Spaces:

The SNF staff looks professional and well groomed.

The temperature of the facility makes the residents feel comfortable.

Staff:

The staff treat each of the residents with warmth, compassion and respect.

All staff have name tags.

There is a full-time Registered Nurse available at all times along with the Director of Nursing.

Residents’ Rooms:

Residents are able to keep their personal belongings along with furniture in their rooms.

Each room has personal storage space which includes a closet and drawers.

Residents have access to a telephone and TV in their room.

Residents are able to choose their own roommate.

Activities:

All residents including those who aren’t able to leave their room are able to participate in activities.

The SNF has outdoor areas available and staff are able to help residents go outside if need be.

Safety & Care:

Each resident receives individual preventative health care including vaccines to keep them healthy.

Residents are able to see their primary care physician if they choose to do so.

Residents and their families can attend care plan meetings for whenever is convenient for them.

The SNF has corrected all deficiencies (Federal or State requirements) on it’s last inspection report.
